You should consider the work life balance. The work life balance even as an attending surgeon is not going to be as good as an attending anesthesiologist. The hours are still longer and is demanding. I wouldn't put too much thought into the years required for residency. Even in anesthesia, you'll need intern year plus CA1-3.

Combined IM/anesthesia is a dumb idea and a waste of time. It may improve your knowledge, but it does not improve your chances of getting a job. It is impractical. You will end up practicing one or the other. If you try to be both a PCP and an anesthesiologist, you will just end up being mediocre at both. In general, call at WashU averages 1x/week. Avg 2 weekend days per month, usually on different weekends. General OR avg probably 60-65hrs/wk. Relationship with the surgery department on a day to day basis is fine. For the most part, very pleasant mid-west surgeons and surgical residents.
